Please review the HIPAA Policy Statement and sign the Acknowledgement of Receipt of Notice Form.

If you are returning for a follow-up appointment, please download and fill out the Patient Encounter Form and bring it to your visit.

E-Mail Service

Our office is pleased to announce the establishment of an email service for our patients.  This service is to be used for the following uses only:

1. Requests for appointments

2. Requests for lab and test results

3. Requests for medication refills

4. Request a referral

5. Request information about routine office policies
