My staff and I would like to make your visit to the doctor’s office as pleasant and cordial as possible.

Our office is currently using the latest computer technology to streamline the process by which your care will be delivered.  This reduces the administrative burden on the staff in the hopes of providing you with more personalized care.

This website is designed to familiarize yourself with our current office policies and practices. General information describing our office hours and location is found under the Office Information tab.  Under the Office Policies tab, there is a description of our prescription, email, and copying policies. Requests for routine referrals and prescription refills can be made by clicking the Online Services tab.  Links to useful medical information sites, St. Clair Hospital, and diets are listed under the Medical Resources tab.

If you are a new patient to the practice, please take a moment to review our current Office Policies and Practices, then proceed to Patient Registration and download the appropriate forms. If you need to have your records transferred to our office, please fill out a medical records release form and mail it to your previous physician.
