Technology

We have spent considerable time streamlining the process by which your care will be delivered.  Our office is currently using the latest computer technology available to reduce the administrative burden on our staff.  Computer systems are in the administration and patient care areas to best serve you and your medical needs.

 

E-Mail Service

Our office is pleased to announce the establishment of an email service for our patients.  This service is to be used for the following uses only:

  1. Requests for appointments

  2. Requests for lab and test results

  3. Requests for medication refills

  4. Request a referral

  5. Request information about routine office policies

Please review the HIPAA Policy Statement and sign the Acknowledgement of Receipt of Notice Form.

Office Policies

Payment for Services

Co-pays are required prior to your visit.

If you are uninsured, please note that Federal Law does not permit discounting physician services. A $40.00 deposit is due prior to your appointment.  If you do not have the deposit, you will be asked to reschedule your appointment.

Payments may be made by check, cash, or credit card.

Prescriptions

Our office will be using electronic prescribing with participating pharmacies for your convenience.

Prescriptions refill requests may be called or e-mailed to us during normal business hours.  Only those requests called in before 3:00PM will be done that day.  Prescriptions called in after 3:00PM will be done the next business day.

If your prescription request was phoned into our office please call your pharmacy prior to picking up your prescription.

If your prescription request was e-mailed , we will reply with a confirmation that your prescription was sent.

Note:  If you are running low on your medication late in the week or prior to a holiday weekend, please call or e-mail your refill request on Fridays.  CALLS FOR REFILLS WILL NOT BE TAKEN AFTER NORMAL BUSINESS HOURS.

 

Copies of Patient Lab Results and Medical Records

If you wish copies of your lab results, we will provide them for you.  under State and federal law, the charge will be as follows:

$1.09 per page for first 20 pages*

$0.82 per page for next 40 pages*

$0.28 per page for any additional pages*

Postage, Shipping, and Delivery is billed at actual cost to send

*A different cap applies for records requests for social security disability purposes.  Please call the office to discuss costs associated.

Forms:

There will be a charge for forms as follows:

There will be no charge for a physical form as part of a physical exam.

There will be a $15.00 charge for any disability form, FMLA form, or letter written on your behalf.

This website will continue to evolve over time.  The next up-date will include any changes to our office policies.  Additionally, I hope to add information about common medical conditions with links to trusted web sites.

Appointment Tips

Make you visit with us more efficient and personalized by following the following tips:

1.  Keep your cell phone off when at the doctors office, it allows us to pay attention to you and your medical needs, and avoids interference with our diagnostic equipment.

2.  Complete as much paperwork prior to your scheduled appointment.  Less time writing on forms means more time for patients.

3.  Keep a list of your current medications and dosages.  A med list makes it easy to remember what you are taking as well as aid in the event of an emergency
