OFFICE POLICIES

Co-pays are required immediately prior to your visit.

 If you are uninsured, please note that Federal Law does not permit discounting physician services.  A $40.00 deposit is due prior to your appointment.  If you do not have the deposit, you will be asked to reschedule your appointment. 

Payments may be made by check, cash, or credit card.

 

Prescriptions

Prescription refell requests may be called to the office , sent by e-mail at office@ragdoc.com, or by using the Online Services Tab.  Only those requests called in before 3:00 p.m. will be addressed that day.  Prescriptions called in after 3:00 p.m. will be done the next business day.

If your prescription request was phoned into our office, please call your pharmacy prior to picking up your prescription.

If your prescription request was emailed, we will reply with confirmation that your prescription was sent.

 Note:  If you are running low on your medication late in the week or prior to a holiday weekend, please call or email your refill request by Friday.  CALLS FOR REFILLS WILL NOT BE TAKEN AFTER NORMAL BUSINESS HOURS.

 

*E-mail Policy

Our office is pleased to announce the establishment of an email service to our patients.  This service is to be used for the following reasons only:

1. Requests for appointments

2. Requests for lab and test results

3. Requests for medication refills

4. Request a referral

5. Request information about routine office policies

 *To take advantage of this service, please read the E-mail Policy Statement and sign the E-mail Agreement Form, which needs to be faxed or mailed to the office.

 

 

 

 

 

 

 

NOTIFICATION OF LABS/X-RAY RESULTS

 

Every effort will be made to notify you promptly of any lab or x-ray study.  Please be advised that there is usually a 24 to 48 hour turnaround time before we receive lab results and x-ray results.  If you have not heard from us in 72 hours after your testing was done, please contact our office.

 

Please note:  if you have a test performed by another physician, it is best to contact that physician’s office for

the results.

 

COPIES OF PATIENT MEDICAL RECORDS

 

If you request copies of your medical records, we will provide them for you.  Per State and Federal law, the charge will be as follows:

            $1.25 per page for the first 20 pages*

            $ .93 per page for the next 40 pages*

            $ . 31 per page for any additional pages*

Postage, shipping and delivery are billed at actual cost to send.

 

*A different charge applies for records requested for Social Security Disability purposes.  Please call the office to discuss the costs associated.

 

FORMS*

 

 A minimum charge of $15.00 will be assessed for completion of disability forms, FMLA, or a brief letter written on your behalf.   Letters which require extensive review of your medical condition will be billed accordingly.

 

There will not be a charge for completion of physical forms at the time of your exam. 

 

*However, a charge may be applied for physical forms that need to be completed after your physical exam is completed.  This does not apply to physicals needed for surgery clearance.
