E-Prescribing

Electronic Prescribing is a means a doctor can prescribe medications directly to your pharmacy. 

An electronic signature verifies the physician, and the pharmacy can process the prescription.

Services

 

Copies of Patient Lab Results and Medical Records

If you wish copies of your lab results, we will provide them for you.  under State and federal law, the charge will be as follows:

$1.09 per page for first 20 pages*

$0.82 per page for next 40 pages*

$0.28 per page for any additional pages*

Postage, Shipping, and Delivery is billed at actual cost to send

*A different cap applies for records requests for social security disability purposes.  Please call the office to discuss costs associated.

E-Mail Service

Our office is pleased to announce the establishment of an email service for our patients.  This service is to be used for the following uses only:

1. Requests for appointments

2. Requests for lab and test results

3. Requests for medication refills

4. Request a referral

5. Request information about routine office policies

To take advantage of this service, please read the E-Mail Policy Statement and sign the E-Mail Agreement Form

We use email to communicate certain information to you should you choose to take advantage of the service.  Note that we do not support your computer and or software on your computer. 

Though all attempts are made to maintain security and encryption, you should not consider any email to be secure or full proof private.
